to complete a new patient registration form and health questionnaire.

For your first consultation we ask that you please bring the following:

Referral from your GP or specialist

Copies of any imaging/X-rays

Medicare card

Private health insurance details

Work cover or TAC number

FEES

A proportion of the consultation fee will be rebatable from Medicare.
Consultation fees should be paid at the time of the consultation by cash, EFTPOS, Visa or Mastercard
(Amex and Diners Card are not accepted).

Please be aware that in most instances, a co-payment is required for surgical procedures.
There may also be extra charges for orthotics.

APPOINTMENT CANCELLATION

If you need to cancel or change an appointment, please provide at least 24 hours notice so we can offer your appointment to someone else.
